Bring these six things.

  • Photo ID and your insurance card
  • A list of current medications and doses
  • The name and location of your pharmacy
  • Any recent lab or imaging reports you have
  • Names of specialists you already see
  • Questions — write them down, all of them

Paperwork before you arrive

New-patient forms, insurance details, and your pharmacy go in from your phone the night before, so the visit starts at the appointment time instead of after fifteen minutes on a clipboard.

Sign a records release in the same form and we'll request your chart from your previous doctor. That transfer typically takes one to three weeks, which is why it's worth starting before the visit rather than after it.

Just sign a release during your visit or via our online forms. Our staff handles the request to your old office; it generally takes 1 to 3 weeks for records to arrive at our San Benito County location.

What is your policy regarding missed appointments?

A $40 fee is applied after a 2nd no-show within 12 months, though this is waived if you call to cancel. We reserve dedicated time for our neighbors in Hollister and Tres Pinos; an empty slot is a missed opportunity for another patient to receive care.

How do I get a refill?

Ask your pharmacy to send the request electronically; most are handled within one business day. Chronic medications are refilled through the next scheduled visit so nobody runs out between appointments.

How are lab results delivered to patients in the San Juan Bautista area?

We notify you via phone or our secure portal, usually within 2 business days of receiving the report. Whether you reside in Tres Pinos or downtown Hollister, our policy is that you should never have to call us to track down your own results.
